Community Care Management

Community Care Management is the out-patient department of Pratt Regional Medical Center. Community Care is located on the second floor of Pratt Regional Medical Center. Our department is staffed with two full-time Registered Nurses, five part-time Registered Nurses and one Certified Nurses Aide. All services provided by CCM require a physician order.

Hours: 8:30 a.m. - 5:00 p.m. Monday through Friday.
